Allied Health Student Onboarding Information

Congratulations on your acceptance and welcome to our campus!  Below are items that you need to complete or become acquainted with to begin your matriculation to LSU Health Shreveport.  Please note that each section is the student’s responsibility for completing each task!   Please click on each link within a section which provides answers to many questions you might need an answer later on. If you have any questions, please contact the Office of the Registrar at (318) 675-5205 or Please also note that you will be receiving information pertaining to your matriculation from the Office of Admissions, School of Allied Health Professions.

Information on how to submit each document is contained in each section or you can upload documents securely by clicking this link Upload Form for our office to retrieve the information.  You may take a photo and save as a jpeg or pdf document to upload!   Please take the time to read each section thoroughly. If you are not able to upload these documents then please return them via mail or fax. Do not email documents to us!

STEP 1:  PERSONAL INFORMATION REQUEST FORM (This form must be submitted first before any communication will initiate from our office)

Please complete the personal information request form by clicking here and submit it online NOW. We must receive this form online NOW to process your information. Please complete this form carefully.  Any field that is incorrect or duplicated will be voided.  Please review each field before you submit the online form.  


Please note that all students are assigned a provisional admission status.  Once our office has received all required documents and have been cleared by respective departments, the student’s admission is then changed to clear for admission. The reason a student may not be cleared for admission includes:

  • All official transcripts from previous colleges have not been received.
  • Immunization items are incomplete (except outstanding repeated vaccines for Hep B; initial repeated dose must be received to be clear)
  • Drug screening results have not cleared by Certiphi Screening
  • Clearance of Federal Healthcare Eligibility form by compliance officer  
  • Clearance regarding Criminal Background Check
  • Receipt of other pre-matriculation forms included in this packet

Computer Requirements:

By accepting your position in the program, you are accepting the responsibility of obtaining a wireless internet capable laptop computer meeting the below the requirements for use throughout your program. Students may request financial assistance to rent or purchase a personal computer by contacting the Financial Aid Office at or (318) 675-5561.

You are required to have a wireless internet capable laptop computer:

  • PC: Any PC capable of running Windows 10 or higher.
  • Mac: Mac OS 10.13 or higher.
  • Your PC or Mac should have the following:
  • Built-in webcam or external webcam with a minimum of 640x480 resolution.
  • Built-in microphone or external microphone/headset/earphones with microphone.
  • At least 6 GB of RAM and 300 GB hard drive.
  • Broadband connection to the Internet (25Mbps minimum, 40Mbps recommended).
  • Microsoft Office (Word, PowerPoint, and Excel). Microsoft Office is free for LSU Health Shreveport students upon email account activation. Students can visit and login with their email address and password. Once logged in click on “Install Office.” This process can be done on up to 5 personal devices.


  • LSU Health Shreveport requires all incoming students to complete a criminal background check through the University vendor. Incoming students assume responsibility for the cost of this service (~$100).  
  • Please submit the online Personal Information Request form (as indicated above) then your request will be entered into the background check database which will initiate an email given details on the ordering process. 
  • Watch for the code LSUHealth to use for the Application Station.  Please give us 36-48 hours to process your Personal Information Request form before you will receive this initial email!
  • The results of the background check are available within 10 days. 
  • Should any questionable, illegal, or negative indicators appear on the check, the School of Allied Health Professions Director of Admissions and Records will confer with the Program Director and Associate Dean for Academic Affairs regarding the most appropriate course of action. Many considerations are weighed including the effect of state and national licensure.

Most administrative business at LSU Health Shreveport is conducted by electronic methods (i.e. email, self-service portal). You must provide your consent by completing the LSU Health Shreveport Student Administration Electronic Consent Form in order to conduct business electronically with LSU Health Shreveport. Please visit this webpage to download the form and submit it to our office. It is imperative you read this section carefully!

Guidelines for Students with Disabilities/Request for Academic Accommodations
The School of Allied Health Professions is committed to providing for the needs of students who are requesting considerations for accommodations. It is the students responsibility, however to review the technical standards for a given program and make their needs known. Newly enrolled students with diagnosed disabilities who are requesting consideration for accommodations may do so by visiting HERE and clicking on “Guidelines for Students with Disabilities” and “Request for Academic Accommodations Form.” Students having specific questions should contact the Office Admissions and Records at, (318) 813-2908 or room 3-342. 


Our campus shares the student information system with LSU Health New Orleans known as PeopleSoft. This portal can be accessed at Instructions to use this system can be found at


All students also must complete a student validation process which requires each student to review the bio/demographic information that was entered into PeopleSoft from your application.  Please note that if any area is incorrect please submit to our office via email the needed corrections, if this is an area that cannot be self-corrected.  When reviewing your prior education institutions, make sure you carefully see each college is listed.  As our office receives official transcripts, we will update corrected degree conferral dates, etc., so please review this panel and proceed with the process until it is complete.   To complete this process, go to this site, log onto the portal and click on the link in your ToDo list on the Student Center tab.  Once the task is successfully completed, it will be removed from your ToDo list and the block is removed from your profile.  More information for this requirement can be found at this link.   Remember, this must be completed BEFORE we can enroll you in courses for the term you are entering. 

STEP 4: STUDENT IMMUNIZATION REQUIREMENTS (requires your immediate attention)

Student Immunization Requirements –We use Certiphi Screening for submission of required immunizations and test using the MyRecordTracker portal. Before you begin this process, you must review the required immunizations and tests listed here  A 2-step PPD requires two separate tests taken and read at different intervals typically within 3 months. Also, if you have to repeat the Hepatitis B series due to a negative titer, please make sure documentation is provided for the repeat initial dose in order for it to be approved. For detailed information on how to use this portal, please click here for the user guide.  Also, if your immunization records are under a different name, you will be required to provide documentation of your current name in order for your records to be approved!

There is no charge for the student to use this portal for uploading your health information.  You will receive a welcome email from MyRecordTracker giving details on setting up your login. Please check your spam/junk folder for your login.  If you have a gmail personal email account and have not received your login please email us. If you have trouble creating your profile, please contact their customer service for assistance. All required health information must be submitted through the MyRecordtracker portal and is reviewed by their medical review team.  Do not submit immunization information directly to our office!   Please pay attention to this NOW and not later!  It takes time to gather this information. Some vaccinations require two doses that are a month apart so please review this form now!  If you do not have a personal physician, there are some work care clinics or total care clinics that are located in the Shreveport area.  Some students also use their college’s student health center. Deadline to upload all required health documentation to the MyRecordTracker portal is May 14, 2021. Do not wait until you have all immunizations to upload them to myRecordTracker portal!  Each immunization requirement is approved and updated individually.  We do not have a form to provide to a physician.  If you cannot locate your immunization records, you may have to obtain titers to determine your immunity for each section for compliance.  Please be advised that if you have to repeat a series of vaccine the initial dose must be documented BEFORE you are cleared for admission!  Our office will determine your compliance on this item. Do not upload any immunizations through the upload form indicated in the first paragraph!  All forms and documentation of immunizations must be uploaded to the MyRecordTracker portal.

Selective Service Compliance (For Men Only): Verify your registration at and print out this page along with returning the Certification form.  We need both forms submitted by May 14, 2021.


Please have your school(s) send to the Office of the Registrar official transcripts from each higher ed institution you have attended or currently attending (this includes all undergraduate and possibly graduate level coursework). Transcripts marked "Issue to student" or submitted by the student are not acceptable. We must receive the transcripts directly from the institution not emailed by the student!  This also includes any college credit you took while enrolled in high school (dual enrollment)!  If you are currently enrolled in school this semester, request that the transcript be sent AFTER the posting of your grades or degree.   The CAS systems do not provide official transcripts to the Office of Admissions when you applied so please make sure you begin this process.  The Office of the Registrar can receive official transcripts via electronically which is our preferred method.  All official transcripts must be received in the Office of Registrar by June 4, 2021.  You may view the status of any transcript via the Student Self Service portal.  Please click on this link to view job aids specifically the Viewing Admission Information for instructions on how to view your external education.


Student Housing – our campus does not offer student housing.  You may visit;or for listings of real estate that may suit your needs.  


Drug Screening Form - Please click here to download a consent form that you must sign and return to our office by Friday, April 30, 2021.  The consent form for the drug screening must be returned to our office before you will receive an email notification from Certiphi Screening in which will direct you details on how to make an appointment for the drug screening. Please see the code LSUSOM-SCHOOLDS to place your drug screening appointment! After you create your profile with Certiphi, you will submit a consent to them. Please note that you only have 14 business days from the date you submit the consent to Certiphi to have the drug testing completed.  If you fail to make your appointment, it will appear as a "no show".  If this occurs, please call Certiphi and request to update your appointment.  It is your responsibility to have this done in a timely manner!  There is no charge on your behalf to have this completed!  Consent forms must be submitted to our office by Friday, April 30, 2021.  All drug screening tests must be completed no later than May 7, 2021. If you are planning to be out of the country, it is your responsibility to care of this. 

Federal Healthcare Eligibility Form – Please complete the form entitled “Federal Healthcare Program Eligibility Information” and return it to our office by Friday, April 30, 2021 This form is included with the consent forms for the drug screening link listed above. The basis for the completion of this form is to determine if any individual has been charged with fraud regarding Medicare or Medicaid.  Everyone must submit this form regardless if this doesn’t pertain to you!

LSU Health Shreveport E-MAIL ACCOUNTS

E-mail accounts– you will be given an e-mail account with LSU Health Shreveport after we receive notification from the Office of Admissions of your acceptance.  Your detailed logon credentials will be sent to you by the Office of Admissions, School of Allied Health Professions.  Please watch your personal email account regularly.  Information to activate your account will be emailed to your personal email account so please check your personal account regularly.  The subject of the email will be LSU Health Shreveport email account activation instructions so please watch for this in case it does go to your spam/junk folder.  If is your student responsibility to check this account regularly.  Once activated, all school communication will be sent to the LSU Health Shreveport email account.


Tuition and fees – Enrollment in courses is done automatically by the Registrar's office since you are in a structured curriculum.  Information regarding financial services can be found on this webpage.

A “Blood Borne Pathogen Insurance Fee” will be assessed to students in the Cardiopulmonary Science (CPS), Medical Laboratory Science (MLS), Physical Therapy (PT), and Physician Assistant (PA) programs. The annual fee is $31. Revenues will be used to provide insurance for students in their clinical portion of their academic program to cover costs associated with needlestick/bloodborne pathogen exposure so students do not have to file claims on their personal health insurance.


Information regarding Parking, Student IDs, and Yearbook photos will be provided to you from the Office of Admissions, School of Allied Health Professions.  Please watch your email for this information!

All requests for name changes must be supplied with supporting documentation i.e. marriage license, divorce decree, social security card, etc.  along with the Name Change Request form found on our website. 


Please refer to this website regarding the school’s student health insurance requirement.  Please watch for an official email from the Registrar about the process coming in early May!  This is just notification of this requirement at this time! All students must have proof of their own policy or can choose to enroll in the United Healthcare policy offered through Gallagher.  Medicaid is acceptable form of a waiver only if issued within the state of Louisiana.  


If you ever need proof of your enrollment, you may email our office for an enrollment verification.  Also, as you transition from one school to ours, you want to make sure any loans for your pre-professional coursework are in deferment; thus, please request an enrollment verification from our office which you can submit to your lender.  Any future notifications to your lender will then be processed through the National Student Clearinghouse.
