Allied Health Student Onboarding Information

Congratulations on your acceptance to our school.  This page outlines pre-onboarding requirements that must be met for our campus.  Please review each step and if you have any questions, please refer them to me, Kim Carmen Registrar directly.

Step 1:  Personal Information Form:  Complete the Personal Information Form to begin the process.  We must populate your bio/demographic in our Student Information System with correct data to begin your admission status.  Thus, please make sure you fill this form out correctly before submission.  If data is incorrect, it may delay processing your enrollment status. If you do not plan to enter our institution, please do not complete the form and reply to this email that you would like to have your acceptance withdrawn so we can update our records in a timely manner.

Step 2 Email account:  Based on the email provided by the Admissions office that you put on your admission application, you should receive an email from SHV ITG Security with the subject:  “New Accounts Activation Instructions”.  If you do not receive this email, please check your spam/junk folder.  If you are already an employee of LSU Health Shreveport, this already exists. If you are an employee or past student of LSU Health New Orleans, then you may already have an existing user name with their system.  Please contact ITG at 318.675.5606 if you do not receive the SHV ITG Security email within a week of receiving this email! This usually means you used your undergraduate school email and it may already be disabled by your school; thus please provide a personal email account. You now use one user id to log onto the PeopleSoft Self service portal!   

Step 3: Student Data Validation -   Before the Registrar’s office can enroll you in courses, you must validate your student information.  You will not be registered for classes until this process is completed.  Please log onto the Student Self Service portal.  Please follow the instructions for Validating Student Information. This process must be completed by Friday, April 19, 2024.

Step 4Criminal Background check: LSU Health Shreveport requires all incoming students to complete a criminal background check through the University vendor. Incoming students assume responsibility for the cost of this service (~$100).  

  • Please submit the online Personal Information Request form (as indicated above) then your request will be entered into the background check database which will initiate an email given details on the ordering process. 
  • Watch for the code LSUHealth to use for the Application Station.  Please give us at least 5 business days to process your Personal Information Request form before you will receive this initial email to your email account you put on the Personal Information Request form!
  • The results of the background check are available within 10 days. 
  • Should any questionable, illegal, or negative indicators appear on the check, the School of Allied Health Professions Director of Admissions and Records will confer with the Program Director and Assistant Dean for Academic Affairs regarding the most appropriate course of action. Many considerations are weighed including the effect of state and national licensure.
  • This must be processed by Friday, April 19, 2024.  We will review the results through the Certiphi portal so you do not need to submit your copy.  Please keep this your pdf copy for future needs such as clinical affiliates when you do the clinical portion of your curriculum.

Step 5: Immunization requirements:  LSU Health Shreveport requires all students to submit the Immunization form with requested documentation using the MyRecordtracker portal through Certiphi Screening.  Access to this portal will be sent to the email account you indicated on the Personal Information Form.  Activate this account and review the Student Immunization Requirements website for accessing the form noting required vaccinations and titers (blood tests) that will be required.  The form must be completed and signed by a healthcare practitioner.   Please try to scan the document as one pdf attachment and link to each section as required.  If test results are required, please include these as supporting documentation.  Refer to the Student Immunization Brochure for assistance to any questions you might have.  Instructions for using MyRecordtracker can be found here!  It is imperative that you have this form processed and uploaded to the portal by Friday, May 3, 2024

Step 6: Conducting Electronic Business: LSU Health Shreveport conducts most administrative processes by electronic methods (self-service portal, email).  In order for us to conduct this business electronically, you must review the information on the Conducting Electronic Business website and submit the form through the Student Self Service portal.   If you choose not to submit this form, all transactions will take place via person at the appropriate office.

Step 7: Certification of Selective Service Compliance: In accordance with the requirements of Louisiana R.S. 17:3151 (Acts 1985, No. 185, Section 1; Acts 1987, No. 214, Section 1; and Acts 1999, No. 345), and the Federal Selective Service Act, all students who are required to register for the selective service under the Federal Military Service Act must provide the institution proof of registration prior to enrolling by submitting the required online form. In general, the students who are required to register are males who are 18 years old or older and who have not yet reached their 26th birthday. For more information go to Who Must Register  Please submit the Certification of Selective Service form by Friday, May 3, 2024

Step 8:  Drug testing process: Please submit the Pre-candidate Authorization form  for consent of drug testing before Friday, April 26, 2024. Once we receive the form we will place the request for an email notification from Certiphi Screening to you which will direct you on how to make an appointment with a location within your vicinity.  There is no cost for the student to have this completed.  Please use the code LSUSOM-SCHOOLDS to place your drug screening appointment.  Instructions for using the Application Station are located here! Once you create your profile with Certiphi, you will submit the Code of Conduct giving consent for a drug screening.  DO NOT DO THIS STEP UNLESS YOU CAN COMPLETE THIS WITHIN THE NEXT 7 DAYS! You are only given a 7 day window after submission of the code of conduct consent to schedule the drug test. 

If you fail to make the appointment, it is returned to us as a “no show” result meaning the next appointment is your expense with an out of pocket cost for $29.  No exceptions are made so please plan accordingly.   All drug screenings must be processed by Friday, May 10, 2024.

Step 9: Official transcripts: Please have your school(s) send to the Office of the Registrar official transcript if you are currently enrolled this Spring term only.  Transcripts marked "Issue to student" or submitted by the student are not acceptable. We must receive the transcripts directly from the institution not emailed by the student!    If you are currently enrolled in school this semester, request that the transcript be sent AFTER the posting of your grades or degree.  The Office of the Registrar can receive official transcripts via electronically which is our preferred method.  Please send to registrar@lsuhs.edu or we are listed as Louisiana State University Health Sciences Center Shreveport - All Admissions through Parchment. All official transcripts must be received in the Office of Registrar by Friday, June 28, 2024.  We receive transcripts you submitted through your CAS application automatically from the Office of Admissions. We will only need completed Spring transcripts only!

Information about Registration Information, PeopleSoft Access, & Misc. stuff

Tuition and fees – Enrollment in courses is done automatically by the Registrar's office since you are in a structured curriculum.  Information regarding payment of tuition and fees will be provided by the Bursar’s office.  Please review the tuition and fee schedules for the Summer 2024 term at the Bursar’s webpage.  Tuition and fees are due on the official first day of class. 

PeopleSoft Access (Student Self Service Portal) -  Our campus shares the student information system with LSU Health New Orleans known as PeopleSoft. A shortcut link can be found under the Student Resources section of the Geaux Inside LSU Health Shreveport page.  Details related to signing in to this portal on and off campus can be found at Academic Self Service Guide by clicking on sign on and off instructions as well as details on Academics, Financial Aid area, Personal Information, Student Financials and Admission data.

Student Housing – our campus does not offer student housing.  You may visit www.shreveport.com www.apartmentsshreveport.com;or www.aptshopper.com for listings of real estate that may suit your needs.

NAME CHANGES - All requests for name changes must be supplied with supporting documentation i.e. marriage license, divorce decree, social security card, etc.  along with the Name Change Request form found on our website. 

STUDENT HEALTH INSURANCE - Please refer to this website regarding the school’s student health insurance requirement.  Please watch for an official email from the Registrar about the process coming in early August.  This is just notification of this requirement at this time! All students must have proof of their own policy or can choose to enroll in the United Healthcare policy offered through Gallagher.  Medicaid is acceptable form of a waiver only if issued within the state of Louisiana.  

VERIFICATION OF ENROLLMENT/IN SCHOOL DEFERMENT FORMS - If you ever need proof of your enrollment, you may email our office for an enrollment verification.  Also, as you transition from one school to ours, you want to make sure any loans for your pre-professional coursework are in deferment; thus, please request an enrollment verification from our office which you can submit to your lender.  Any future notifications to your lender will then be processed through the National Student Clearinghouse. 

FAQs

1. My checklist indicates "Admission Status Not Cleared".  What does this mean?:  Once the background check is processed and clear; drug screening results are processed and clear; and immunizations are submitted and processed then our office will clear your admission status.  Please allow time once this action is completed for the Office of Financial Aid to remove the item from your checklist.

2.  Do I need to submit any transcripts?  The Office of the Registrar only needs official transcripts from outstanding coursework since your CAS application was certified and verified, eg., if you submitted your CAS application in August and was verified then we may need final transcripts for the Fall term. This also goes for degree conferrals in December 2023.  

3.  During my student validation process, my external education coursework was incorrect.  What do I do?  Please continue with the process.  We are in the process of updating your transcripts so acknowledge the page and continue with the student validation process.

4.  I am having issues with logging onto the Student Self Service portal or I haven't received my email activation.  Please contact ITG Help Desk at 318.675.6506 for assistance with your account.
